Charles O. Forsyth served in the Union Army of the American Civil War. He was mustered in on September 11, 1862, at Camp Anna and served as a Corporal in Company K, 109 IL US Inf. He was then assigned as Corporal to Company F, 11 IL US Inf. He was promoted to Sergeant on March 6, 1864, and then to First Sergeant on January 1, 1865. He was mustered out on July 14, 1865, in Baton Rouge, LA. His Civil War Pension Index Card, Application No. 270,195, dated March 3, 1879, is marked "Dead." His record of burial lists the cemetery as "Villa Ridge" and is stamped "Lee Marble Works Contract June 10, 1903."
