BLIND APPLICATION
For Relief by Blind Soldiers, under Act Approved February 13, 1891
State of Alabama
County of Blount
Personally appeared before me, J W Ellis, Judge of Probate in and for said county, Joseph H Jett, who being duly sworn and saith, that when a 3rd Lieutenant in Company F of the 29th Ala Regiment of Inf and while discharged of his duty on __ day of __ 1864 at Pensacolla in the state of Fla he was ordered on a force march and his eyes got sore from overheat and finally one went out now the other is out from same cause and that in consequence of which wound was received or disease contracted in said service has lost his sight; and is a resident of this state of application; that he does not own taxable property of the unencumbered value of $1000.00 or gross income or salary over the sum of $1000.00 per year.
Sworn and subscribed before me this 13th day of June 1891.
